The four core functions of the Primary Care Trust are to: - Improve health and reduce inequalities in the local population
- Develop primary care (eg practice) based services
- Commission (or purchase) hospital and other health services
- Provide community and rehabilitation based services
Our commitment to meeting these functions is reflected through the following aims: - To improve our planning and performance management processes
- To ensure effective governance arrangements are in place, including liaison with stakeholders
- To integrate health and social care services, where appropriate
We are taking forward these aims through eight key objectives, which were set out in the Primary Care Trust consultation paper ‘Improving Health Services’ in January 2000: - To improve the levels of primary and community nursing services
- To fully integrating nursing and
social services teams within a practice or community area
- To develop intermediate care facilities for people who do not need hospital based services on a long-term basis
- To create community-based rehabilitation services, enabling people to return to their own homes as soon as is practical and appropriate
- To provide access to a wider variety of proven clinically effective services in GP surgeries and within the local community
- To give people quicker access to the health services they need
- To streamline booking arrangements for health services
- To encourage greater participation of other local organisations and the public in health services
Good progress has already been made against these objectives and progress will continue to be measured and reviewed by the Primary Care Trust Board, in conjunction with local stakeholders, including the community of South Wiltshire.
